The Gary Burnstein Community Health Clinic provides high-quality, compassionate medical and dental care, social services, education, outreach to men, women and children in need.
COMPREHENSIVE MEDICAL CARE
All of the following services are provided free to all patients at the GBCHC:
• basic primary care
• patient education
• disease prevention & screening
• women's health, including gynecological care
• men's health
• laboratory testing
• gynecological care
•help with attaining free medications through the Patient Assistant Program
PATIENT REQUIREMENTS
To apply as a patient you must:
- Supply your most recent financial information (tax return, W-2, or 4506-T)
- Supply a valid picture ID (drivers license, state ID, prison ID, or green card)
- Be an Oakland County resident
- Fall within our income guidelines
- Be uninsured
